Welcome to the 住宅生活 医疗设施 website! 这个过程是为了 住宅 Students who live 校园 and are in need of a Medical Accommodation and/or an Emotional Support Animal. 的 住宅生活 process does not encompass any academic accommodations. 这个网站 was created with our students’ well-being in mind and with the purpose of transparency as the university is committed to providing a healthy, inclusive and safe environment that supports the growth and development of all students.
How to Request a Medical Accommodation and/or an Emotional Support Animal

To apply for a Medical Accommodation and/or Emotional Support Animal, students should work with their medical providers to complete the documentation included in the form 下面. Emotional Support Animals require additional paperwork, such as vet records, roommate agreements, and Emergency 联系s. All forms needed for either process are included and outlined in the "医疗设施 Portal" 下面.
Emotional Support Animals (ESAs)
Emotional Support Animals (ESAs)- the ESA policy document will be updated the first week of classes.
Requests for the 23-24 Academic Year are OPEN
的 Office of 住宅生活 reviews all Emotional Support Animal or ESA的应用程序. Any ESA questions can be directed to 住宅生活 at housing@dektinary.com.
Please keep in mind: any unapproved animal on campus will result in a student fine. ESAs must be approved before animals are brought to campus. 校园动物 prior to ESA approval may result in a denial of the application. ESA的应用程序 are reviewed on a weekly basis.
Before submitting an application, please review the 住宅生活 ESA 政策. If approved, students will review and sign with their building coordinator.
Any animal that is referenced in the ESA packet must be AT LEAST 9 months at the time 关于请求.
If a student was approved for an ESA in the previous year, they will need to re-apply by submitting an new Roommate Agreement and updated Vet Records.

This process is open for the 23-24学年. All requests are reviewed on a rolling basis during weekly committee meetings.
To apply for a 住宅生活 Medical Accommodation, documentation must be filled out by a licensed medical processional and provided to the JU student. 学生 should submit the documentation and any other pertinent information directly to 住宅 Life through the Medical Accommodation submission portal found above.
的 form needs to be completed in its entirety. All requests for 住宅生活 医疗设施 are reviewed by a Professional staff committee. 住宅 Life reserves the right to request updated information, deny incomplete documentation, and conduct a yearly accommodation renewal process.
学生 will receive confirmation of received documents from the 住宅生活 Office initiating the Accommodation Review Process.
拒绝理由:
- Lacking documentation to support request for accommodation
- Student's current placement or living arrangement fulfills Medical Provider's Recommendation
- Student did not fully participate in the Accommodation requests process
When Accommodations are approved:
- Students will receive an email to their JU email address with the offered accommodation within 7-10 business days of receipt of request. (Timeline subject to change during busy seasons)
Examples of Accommodations:
- ADA-compliant浴室
- Semi-private bathroom or a bedroom in close proximity to a bathroom
- 私人卧室
- 私人卧室 with private bathroom
- Placement with preferred roommate
- Ground floor or first floor room
- Residence hall with a dining service option within the same building or in proximity
- Relocation to a different resident hall in order to fulfill a need
- Emotional Support Animal (ESA- see section above)
